Ashwagandha (Withania somnifera) is a shrub whose root is used in Ayurvedic medicine, where it is classed as a rasayana, and it is described in the supplement literature as an adaptogen, a substance studied for its effect on the stress response rather than on a single receptor [1, 2]. The best-supported physiological effect in humans involves the hypothalamic-pituitary-adrenal (HPA) axis, the hormonal pathway that ends in cortisol release. In a double-blind, placebo-controlled trial of 64 adults with a history of chronic stress, 300 mg of root extract twice daily for 60 days lowered scores on all stress-assessment scales (P < 0.0001) and substantially reduced serum cortisol (P = 0.0006) compared with placebo [2]. Because stress and heightened arousal are features of insomnia, this pathway is the proposed route to an effect on sleep.
